Twogether Consulting is happy to have Nicole Wilson, the CEO, and creator of an amazing app that will help both individuals and their guardians or family members as well as providers with finding placement in a residential setting and in tracking their progress.  This app is also for persons finding alternative placements besides ICF and HCS programs.  You may need placement for nursing home or assisted living or possibly personal care homes as well and much more. Nicole’s app also helps with connecting individuals and providers to necessary resources concerning placement and keeps all parties informed during the process. Nicole will discuss some of the solutions they offer providers of services as well as individuals seeking placement and how they can keep up with the progress of this placement process. This session may be of particular interest to those of you who are ICF or HCS providers or persons interested in providing host home/companion care services who may be looking for individuals who want to be placed in your home/s.
From Nicole:
Why I created the App:
“In my 15 years as a Home and Community-based Services (HCS) provider, the question was, how could I make it easier for families to find quality homes for their loved ones, providers to display their resources, and caseworkers to access a network of providers? The answer came in the form of technology: an innovative app that could bridge the gap and facilitate connections within the mental health industry.
This app was designed to simplify the process of finding and accessing mental health resources. It allows families to find quality homes for their loved ones with just a few clicks, and providers and caseworkers to display their resources, access a provider network, and receive referrals conveniently.  The app bridges the gap between individuals, families, providers, and resources, effectively addressing the issues that once hampered the efficiency of Provider Fairs. Its design considers the diverse needs of its users, providing a solution that is not only convenient but also affordable.”

Session Discussion

The Texas IDD billing process through TMHP has been burdening providers since its inception. HCS, TxHmL and ICF providers have been plagued with denied claims, decreased revenue, and inefficiencies in tracking and reporting.
In this webinar, Millin will address a range of best-practice topics to help you stay on top of your Texas HCS, TxHmL and ICF billing. We’ll share the functionality we’ve built into our MillinPro software and our internal process that has led us to an incredible 99.2% collection rate on all eligible claims.
Millin aims to simplify your billing process by bypassing much of the manual work you need to do on TMHP.
Millin aims to automate most of the process with features that include eligibility checks, batch submission capabilities, issue-tracking and a library of reports designed to replace the R+S functions of TMHP.

Presenters:

Description:  This class is designed for new and current HCS/TxHmL providers, especially staff involved in providing care coordination or writing the Implementation Plans for the HCS/TxHmL provider (i.e. Program Manager, Case Manager/Care Coordinator, and Nurses).  This is Part II of our Care Coordination Series. This webinar can also be helpful for new Service Coordinators (SC’s) at the Local IDD Authorities (LIDDAS). This training is appropriate for anyone who will be part of the team planning and implementing services for the individuals in your program. Twogether Consulting developed this session originally as part of a live training done with HHSC, as well as some of the LIDDAS, and over the past 7 years or so, we have adapted to be a  part of our Care Coordination/Case Management Series via webinar and for our live presentations.
This is a great session for those providing oversight and quality assurance for these programs as well. This webinar was developed with the goal of assisting providers in understanding how to develop an individual’s Implementation Plan (IP) and how to do that using person-centered practices. We also wanted to ensure the  HCS & TxHmL providers and Service Coordinators understood how the PDP relates to the IP and what the purpose for each one of these documents is in terms of the individual’s overall “plan” for what they “desire” and/or “need”.

Topics

Balancing Wants With Needs of The Individual: Person-Centered Practices

-Who is Responsible For PDP?

-Definition & Purpose of PDP

–How Does The PDP Relate To The IP

Utilizing “Action Plans” From PDP

-Who is Responsible For IP?

-Definition & Purpose of IP

-How Do I Develop & Document The IP?

(Objectives/Strategies/Justifications of Unit Amounts)

-What Is The Role of The Nurse in IP Development?

Samples of Person-Centered  IP’s

-How To Complete an IPC (Individual Plan of Care)

-What Are Back-Up Plans?

-Creating “Back Up” Plans

Samples of “Back-Up” Plans

–When Do I Revise the IP?
